REQUIRED CURRENT VACCINATIONS & SUCH

  • Rabies 1 or 3 year or exemption letter from Vet
  • DHPP
  • Bordetella Vaccine
  • Centrifugal Fecal Float - NONE SEEN Result
  • SNAP ELISA Test for Giardiasis - NEGATIVE Result
  • All dog's over 6 months must be spayed/neutered

Board and Train Agreement - New Clients or if requested/out of date

  1. Fill-in online, complete all fields, save to your computer and send to us as an email attachment - OR -
  2. Complete all fields on-line and Print out the form
  3. Initial all highlighted sections
  4. Sign the document Keep a copy for yourself and hand the original to your driver or leave with the office at the time of drop-off
  5. Complete the Boarding Reservation Form using the link below. (Until we receive this form, your reservation is not confirmed)

On-Line Boarding Reservation Form - This FORM is required from each client for each visit

  • Once we receive your boarding reservation request, we will verify we have current required documents on file for your dog. We will contact you if we do not.
